Before reporting to Camp on June 4, you must turn in:
1. Complete Medical Form. Must have by Staff Week, June 4.
A BSA Class I and II Medical Form, revised within the past 12 months and a physical examination conducted within the past 36 months, are required of each camp staff under 40 years old. A copy of the physical form must be attached to the health history. A parent or guardian must attest to the validity of the health history and physical examination for those under 18 years old.
A BSA Class III Medical Form, showing evidence of physical examination within the past 12 months, must be completed by adults over 40 years and High Adventure Camp Staff.
2. Completed Venture Crew Application (20 yrs and younger)

Your payroll will be NOT processed until all of these forms are completed and turned in to Camp Director or Camp Program Director.
Complete these forms and (1) bring to staff orientation or (2) bring to staff development week or (3) mail to the Scout office. (address below)
